Dental implants are considered the premier option for tooth replacement. While all implant procedures follow the same general concept — a titanium post surgically inserted into the jawbone with an attached life-like crown — the installation process can vary.
From their earliest history, implants have usually been installed through a two-stage process. In the first stage, the surgeon inserts the titanium post in the bone and leaves it “submerged” below the gum level to protect it from oral bacteria and the effects of chewing and biting. About three months later after the bone attaches to the titanium (a process called osseointegration), the surgeon then performs the second stage by re-exposing the implant and attaching a temporary abutment and crown for the patient to wear while the permanent abutment and crown are fabricated and later attached in 2-6 weeks.
In recent years, advancements in materials and design have made possible a one-stage process that allows the implant to protrude above the gum line during osseointegration and shortens the process. After the initial three-month healing period, the implant is ready for “loading” with the permanent crown.
The choice between which of these two procedures should be used for your implants will first depend on the type of tooth being replaced. A front tooth benefits from the one-stage procedure for cosmetic reasons because the surgeon can install a temporary crown to the exposed abutment during osseointegration (as long as the temporary tooth isn’t in functional contact with other teeth). An implant for a back tooth, on the other hand, doesn't have a large cosmetic demand so those one stage procedures usually end up with an exposed healing abutment but no temporary crown.
The strength of the bone is also a factor. Some bone tends to be softer, particularly in the back of the mouth. There’s a chance the implant could move in this softer bone, adversely affecting the outcome. For this reason, the two-stage procedure can be the preferred approach for posterior teeth as it offers more protection from movement.

One of the unfortunate aspects of aging is tooth wear. Depending on your diet, years of biting and chewing can cause enamel along the biting surfaces to erode. Your body also can't replace enamel — so when it comes to teeth it's not a question of if, but how much your teeth will wear during your lifetime.
To make matters worse, certain conditions cause tooth wear to accelerate. Teeth softened by acids or tooth decay, for example, erode faster than healthier teeth. So will grinding habits: often fueled by stress, these include chewing on hard items like nails, pencils or bobby pins.
You may also grind your teeth, usually while you sleep. Normal biting and chewing produces pressure of about 13 to 23 pounds per square inch: grinding your teeth at night can well exceed this, even up into the hundreds of pounds.
There are some things we can do to alleviate these issues. For clenching and grinding habits, one primary step is to address stress through counseling or biofeedback therapy. For nighttime teeth grinding we can create a bite guard to wear while you sleep that will prevent your teeth from generating abnormal forces.
Finally, it's important that you take care of your teeth through daily oral hygiene, regular office cleanings and checkups, and a nutritious diet for maintaining strong bones and teeth. Keeping your teeth free from diseases that could compromise your enamel as well as other aspects of your mouth will help them stay as strong as possible.

If you were asked to identify the number one mouth problem affecting dental health, what would you name? Toothaches? Poor hygiene? Jaw joint issues?
Believe it or not, the top issue among 15,000 respondents in a recent American Dental Association (ADA) survey was dry mouth. A full one-third of the respondents had experienced chronic lack of normal saliva flow; difficulty biting and tooth pain, took second and third place, respectively.
We’ve all experienced the discomfort of temporary dry mouth when we first wake up in the morning or after eating certain foods. But chronic dry mouth is much more serious with long-term effects on a person’s teeth and gum health. This is because among its other important properties, saliva helps neutralize enamel-softening mouth acid and restores minerals to enamel after acid contact. Without sufficient saliva flow you’re much more susceptible to dental disease.
While there are several causes for dry mouth, perhaps the most common is as a side effect to at least five hundred known medications. Because older people tend to take more medications than other age groups, dry mouth is an acute problem among people over 60 (a major factor for why dry mouth took the survey’s top health problem spot).
You can help ease dry mouth from medications by first asking your doctor about switching to alternative medications that don’t affect saliva production. If not, be sure to drink more water during the day and especially when you take your oral medication (a few sips before and after).
You can help your dry mouth symptoms from any cause by drinking more water, limiting your consumption of alcohol or caffeine, and avoiding tobacco products. You can also use substances that stimulate saliva flow—a common one is xylitol, an alcohol-based sugar that’s used as a sweetener in certain gums and candies. Not only does xylitol boost saliva flow it also inhibits the growth of bacteria and thus decreases your risk of disease.
And speaking of reducing bacteria and their effects, don’t neglect daily brushing and flossing. These habits, along with regular dental cleanings and checkups, will benefit you just as much as your efforts to reduce dry mouth in avoiding dental disease.
Brushing and flossing every day are important for preventing dental disease. The object is to remove as much bacterial plaque, the thin biofilm most responsible for disease, from your teeth and gums as possible.
But how do you know your hygiene efforts are that effective? You can of course do the “tongue test” â?? feel your teeth with your tongue after brushing and flossing and see if they feel smooth and “squeaky” clean. We can also give you a “report card” at your regular cleaning appointment. There is, however, a more precise way you can find out at home by using a plaque disclosing agent.
A plaque disclosing agent is a formulation that when applied to the teeth will temporarily dye any bacterial plaque present a distinct color. While dental hygienists occasionally apply them, they’re also available over the counter for use at home. They’re usually found in a solution, tablet or swab form in various flavors.
To use the product you first brush and floss, then swish the disclosing liquid around in your mouth for about 30 seconds before spitting it out (or chew the tablet or apply the swab as directed). The agent will react with any remaining plaque and dye it a bright color. There are even two-tone agents available that can differentiate between old and new plaque and dye them different colors.
Examining your teeth in a mirror will give you a good idea where you need to concentrate your attention when brushing and flossing. If, for example, you see dyed plaque more along the gum line and less in other places, then that’s where you should focus your hygiene efforts.
While the dye will eventually wear off on its own, you should take the opportunity to brush and floss again to remove any remaining dyed plaque. Not only does this provide a more thorough cleaning at that moment, you’ll also get a better sense of how “thoroughness” feels for future brushing and flossing.
It’s always good to know how well you’re doing with your dental hygiene efforts. A plaque disclosing agent can give you just the right feedback to help you improve.

While the prevention and treatment of tooth decay has improved dramatically over the last half century, it continues to be a major health issue, especially for children. One in four children 5 and younger will develop some form of the disease.
Although tooth decay in children stems from the same causes as in adults — the presence of decay-causing bacteria in plaque, unprotected teeth and the right mix of carbohydrates like sugar left in the mouth — the means by which it occurs may be different. We even define tooth decay differently in children as Early Childhood Caries (ECC), “caries” the dental profession’s term for tooth decay.
ECC highlights a number of cause factors specific to young children, such as: continuous use of a bottle or “sippy cup” filled with juice or other sweetened beverages; at-will breast-feeding throughout the night; use of a sweetened pacifier; or regular use of sugar-based oral medicine to treat chronic illness.
If you noticed sugar as a common denominator in these factors, you’re right. As a primary food source for bacteria, refined sugar is a major trigger for the disease especially if it constantly resides in the mouth from constant snacking or sipping. In fact, it’s the primary driver for a particular pattern of decay known as Baby Bottle Tooth Decay (BBTD). This pattern is specifically linked to sleep-time bottles filled with juice, milk, formula or other sweetened beverages, given to an infant or toddler to help soothe them through the night or during naps.
All these factors cause a cycle of decay. To interrupt that cycle, there are some things you as a parent should do: perform daily hygiene with your child to reduce decay-causing bacteria; reduce the amount and frequency of carbohydrates in the diet, particularly sugar; and protect the teeth by having us apply fluoride or sealants directly to the teeth.
Early tooth decay could affect your child's oral health for years to come. With a little care and vigilance, you improve your chances of avoiding that encounter.
